Caregiver - Phoenix AZ, PRN

Caregivers at TrueComfort Home Healthcare Solutions LLC provide non-medical, in-home support to help clients with daily living activities, light housekeeping, meal preparation, and companionship. Key Responsibilities: 

  •  Follow the plan of care to deliver safe, competent care to clients. 
  • Assist clients with personal hygiene and daily living tasks. 
  •  Plan and prepare nutritious meals for clients. 
  •  Encourage clients to remain as independent as possible according to their care plan. 
  •  Provide emotional and psychological support to clients and their families. 
  •  Observe and report any changes in the client’s mental or physical condition. 
  •  Perform light housekeeping to maintain a clean and safe environment. 
  •  Maintain detailed and timely visit reports and client records. 
  •  Collaborate with other community agencies involved in client care. 
  •  Communicate regularly with supervisors about client status and any changes. 
  •  Attend in-service training as required. Job Requirements: 
  •  Must have adequate transportation to travel between client locations. 
  •  Ability to drive and transport clients safely; access to a reliable vehicle. 
  •  Physical ability to assist clients in and out of vehicles and homes. 
  •  Capable of lifting and carrying up to 30 pounds; occasional movement of client weight up to 200 pounds. 
  •  Strong communication skills, both verbal and written. 
  •  Use of basic medical equipment (e.g., BP cuff, thermometer). 
  •  Must respect and maintain client confidentiality. 

Qualifications: 

  •  TB test 
  •  CPR and First Aid certification 
  •  Fingerprint clearance card 
  •  High school diploma or equivalent preferred 
  •  Basic knowledge of personal care, housekeeping, and meal preparation

CNA - Phoenix AZ, PRN

CNAs at TrueComfort Home Healthcare Solutions LLC provide hands-on, non-medical and basic medical assistance to clients in their homes under the supervision of a licensed nurse or care manager. CNAs help clients with personal care, monitor vital signs, and support daily activities to promote comfort, safety, and independence.  If interested, please send your resume to careers@truecomforthealth.com.

Key Responsibilities: 

  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting. 
  • Help with safe transfers, mobility assistance, and positioning to prevent injury and bedsores. 
  • Take and record vital signs (temperature, blood pressure, pulse, respiration) as directed. 
  • Provide medication reminders and assist with medication issuance per Arizona state regulations and agency policy. 
  • Provide companionship and emotional support to clients and families. 
  • Support meal preparation, feeding assistance, and monitor dietary needs. 
  • Perform light housekeeping to maintain a clean and safe environment. 
  • Follow individualized care plans and document services provided. 
  • Report changes in client condition or medication compliance to supervisors or nurses promptly. 
  • Maintain confidentiality and comply with HIPAA and Arizona ADHS standards. 
  • Use proper infection control and universal precautions at all times.

Qualifications:
